Client issue and complaint record. A printable record for a client complaint or issue — what was raised, what was found, what was done, and whether it was the clean or the contract that was wrong.
This is the paper version. CleanOptix does the same job on a phone, with the record filed against the site and the cleaner automatically.

What is on the sheet
- Client, site, who raised it, when and how
- The issue as the client described it, in their words
- What was found on investigation, including whether the work was in the specification at all
- Immediate action taken, and when the client was told
- Root cause — standard, frequency, access, staffing, equipment or specification
- Actions with owners and dates, whether the specification changed, and a close-out signed by the client
Why this record matters
Most complaints on a cleaning contract are about one of three things: work that was not done, work that was done badly, or work that was never in the contract. Those need completely different responses, and a complaint log that does not separate them produces a monthly report full of apologies for things the contract never covered.
So the investigation section asks first whether the work was in the specification. If it was not, the answer is a variation conversation rather than a performance one — and handling that as a failure teaches a client that anything they ask for is included.
Recording when the client was told, separately from what was done, is the field that protects a contract. Clients forgive problems and remember silence, and the gap between the complaint and the first response is what a contract review actually turns on.
The close-out signature is asked for because an issue the contractor considers closed and the client does not is the one that reappears at renewal, with eleven others, as evidence of a pattern.
